Polk County, OR – The authorities have identified the person who died after colliding with a farming swather on Highway 194 near Monmouth at about 6:38 a.m. on June 17, 2025.
OSP identified the victim as Erika Margit Smith, 59, of Dallas.
Smith was pronounced dead at the scene.

Polk County, OR – One person was killed in a two-vehicle traffic collision on Highway 194 in Polk County Tuesday morning.
The deadly crash happened at about 6:38 a.m. on June 17, 2025, on Highway 194 near milepost 5 near Monmouth
According to Oregon State Police, a farming swather was heading westbound on Highway 194 when its boom was struck by an eastbound white Toyota 4Runner. The impact caused the 4-Runner to leave the roadway, coming to rest in the trees on the north shoulder.
The driver of the 4-Runner was pronounced dead at the scene.
No one else was injured in the crash.
The incident remains under investigation.
